95 South to Exit 25 for Smithfield Ave. Left onto Smithfield Ave; follow end (road wraps to right behind Brooks Drugs). At stop, turn left onto Caroline Street. Right onto North Main St. Left onto 2nd Street. Summit Medical Building @ 100 Highland Ave is second building on right.
From 146 South

Saturdays & Sundays are by appointment for sick visits, please call between 8 am and 9 am, as our weekend hours are based on demand.
